A 67-year-old woman presents with a fever, shortness of breath and a productive cough. She has left basal crackles on examination. You make a diagnosis of pneumonia. Her observations are as follows: heart rate 96 bpm, respiratory rate 23/min, BP 133/76 mmHg. Her AMTS is 10/10 and a set of bloods are taken, which reveal her urea to be 6.5 mmol/l.
